Air Traffic Control Procedures in Cities with Multiple Airports

During my visit to Chicago, I witnessed a plane departing from MDW fly underneath another plane that appeared to be heading towards ORD. Is this a common occurrence?
I've been intrigued by air traffic control operations, specifically in Chicago and NYC, but I've been unable to find city-specific information.
Do any of you have insights into the procedures implemented by cities with multiple airports to prevent aircraft from one airport crossing paths with those from another?
